(Undated) – If you received your first dose of the Moderna Covid-19 vaccine on February 3rd the CCHD wants to see you today. According to the Crawford County Health Department, they will host a clinic for those patients that need to receive the second dose today at the Robinson Community Center. You are asked to bring your completed Moderna consent form. This form can be printed from the CCHD website cchd.net or you can pick up a copy from the pickup box outside the front door of the health department. You are asked to also bring the vaccine record card you received at your first appointment so it can be updated with your 2nd dose information. Again, today’s clinic is for those who received the first dose of the Moderna COVID-19 vaccine on February 3rd. If you are scheduled for your first dose please bring your completed consent form and proof that you are in phase 1A or 1B.
