(Undated) — The Crawford County Health Department is inviting parents to “Get a Jump on Back-to-School Vaccines.” According to the CCHD, children entering Kindergarten, 6th, or 12th grade are due for required childhood immunizations. They say appointments are required, so schedule early. Parents and guardians are reminded to bring their current insurance card, their child’s immunization record, and, if you have one, the parental/guardian consent form, which is required if your child is under 18 years of age. The CCHD says that if a parent cannot be present, a dated and signed note from a parent giving permission for his or her child to receive immunizations can be accepted.
