(Undated) — A collaborative program several years in the making is now accepting applications. Lincoln Trail College and Crawford Memorial Hospital are launching a new “Community Health Coaching Program.”
That was LTC Coordinator of Marketing and Public Information, Chris Forde. Forde says the program is designed to give students real-world experience in healthcare while offering crucial support to patients managing chronic health conditions in Crawford County. The CHCP is open to students with a 3.0 GPA who have graduated from high school by June 4, 2025. Applications for the inaugural cohort are due May 2nd.
